New Patient Registration
The Practice will now only accept new patients from the following areas: G41.2,3,4, G43.1,2 & 3.
These are the postcodes within our Practice boundaries and we cannot accept patients that live outwith these areas.
In order to register with the practice you will be asked to fill in a registration form which we will then send to the health board to request your records from your previous practice.
You will then be asked to make an appointment with the practice nurse for a screening check. This is helpful to you and us, as it allows a basic health check and a record of your past medical problems until we receive your medical records from your previous GP practice.
An interpreting service can be arranged for asylum seeker patients.
Medical treatment is available from the date of registration. Please contact reception for further information.

Please inform us of any change to the above as soon as possible. It is very important that the practise has your new telephone number as we may have to contact you at short notice to re-arrange your appointment.
